Free T3 measures the unbound portion of triiodothyronine, the most biologically active thyroid hormone. Most T3 is not made by the thyroid directly but converted from T4 in other tissues, so this marker reflects how well that conversion is working.

What Free T3 measures
Your thyroid produces mostly T4. The liver, kidneys, and other tissues then convert that T4 into T3, which is several times more potent and does most of the actual work at the cellular level. Only a small unbound fraction circulates free and available to enter cells, and that is what this test measures.
The distinction from Free T4 is what makes this marker useful. Free T4 shows what the thyroid gland is producing. Free T3 shows how much active hormone tissues are receiving after conversion. Those two can diverge, and when they do, it usually points to something happening outside the thyroid itself.
What high or low levels mean
Free T3 is not part of a routine thyroid screen. TSH and Free T4 detect most thyroid conditions on their own, and Free T3 is added when those results leave a question open.
High Free T3 can reflect hyperthyroidism or thyroid medication containing T3. In some cases of hyperthyroidism, T3 rises before T4 does, so it can clarify a picture where TSH is suppressed but Free T4 still reads normal.
Low Free T3 can reflect hypothyroidism, but the more common explanation is non-thyroidal illness. During acute illness, surgery, major stress, or significant calorie restriction, the body deliberately slows T4 to T3 conversion to conserve energy. In that setting a low Free T3 is an appropriate adaptation rather than a thyroid problem, which is why the result needs clinical context.

Why isn't Free T3 on every thyroid panel?
TSH and Free T4 identify the large majority of thyroid conditions. Free T3 adds value in specific situations, such as suspected hyperthyroidism with a normal Free T4 or monitoring T3-containing medication, so it is generally added when indicated rather than run by default.
My Free T3 is low but my TSH is normal. Should I be worried?
Often not. Low T3 with a normal TSH is a recognized pattern during illness, stress, or calorie restriction, where conversion slows temporarily. Retesting once you are well typically gives a clearer answer.
Does this test require fasting?
No. Fasting is not required, though prolonged fasting or significant calorie restriction can lower T3, so testing during a period of normal eating gives a more representative result.
